Mixed Martial Arts in Oregon is regulated by the Oregon State Athletic Commission (OSAC).
You must register with a national registry and receive an official MMA card. An application for the MMA card is available at National MMA ID Card Application Form. There is no fee to register. Submit the completed form to OSAC at 3400 State Street, Suite G-750, Salem, OR 97301 either by mail or in person. You must include two forms of legal identification and a passport sized (approx. 2in X 2in) photo. Your national identification card is valid for 5 years.
All fighters must have blood work done before they can fight. You will be tested for Hepatitis B, Hepatitis C and HIV. Test results on all three must be negative. Blood test results are good for a period of one year. Call to Request a Test at 1-888-732-2348 for locations and fast results. The testing location must FAX a copy of the results to Midtown MMA at 541-746-7376 and to the Oregon State Athletic Commission at 503-378-6878. Call the Commission at 503-378-8739 to make sure the test results were received okay.
Professional mixed martial arts fighters must have an annual physical examination in addition to blood work. Click on link for a copy of the official Report of Physical Examination required by OSAC.
